NJ law requires coverage of a medically necessary brace for people on NJ state-regulated insurance — SHBP, SEHBP, and NJ-based ACA/marketplace plans. Different rules apply to large-employer self-funded (ERISA) plans, which aren't bound by the NJ mandate. Please note: we don't bill Medicare or Medicaid for the brace.
