New Jersey's medically-necessary orthotic mandate

New Jersey requires many health plans to cover medically necessary orthotic and prosthetic appliances on terms comparable to other medical services. The relevant provisions are found at N.J.S.A. 17B:27-46.1 (group and individual health insurance) and N.J.S.A. 17:48-6 (hospital service corporation contracts). In plain language:
